Game Play Liability Waiver

By signing below, you, as the customer, acknowledge and agree to the following: 

I, the undersigned, acknowledge and agree to abide by the rules and expectations set forth by The AzucarMa Experience. I further acknowledge that alcoholic beverages are not permitted on the premises and that respectful, sportsmanlike conduct is expected at all times. I understand that roughhousing, physical altercations, and any form of disruptive behavior are strictly prohibited. I affirm that all games and equipment were received in good condition at the start of my session. Rage quitting and aggressive conduct toward others, staff, or equipment are strictly prohibited and could result in immediate dismissal from your session without reimbursement. I accept full responsibility for any damage caused by myself or my party during the course of gameplay. The AzucarMa Experience & it's staff are not liable for ordinary accidents, injury, or personal property loss that occurs during participation. Participants acknowledge that shared recreational spaces may involve movement around furniture, games,and other guests. By signing this document, I agree to take full responsibility for my actions and behavior while on the premises and acknowledge that failure to comply with these guidelines may result in immediate termination of my session without refund.

I hereby grant The AzucarMa Experience permission to use photographs and/or video recordings taken of me during events or while on the premises for promotional purposes. These materials may be used in publications, marketing materials, social media, and the official AzucarMa Experience website, without compensation or further approval. I understand that these images may be used in a variety of formats and platforms and that The AzucarMa Experience will not intentionally use any image or video in a way that is defamatory or harmful. I acknowledge that I am not entitled to any form of compensation for the use of these media materials and that all rights to the images and recordings belong to The AzucarMa Experience. I am 18 years old or older and I consent to the above.
